1.   The notifying entity shall submit to the competent authority its notification by filling in the form set out in the Annex of this Regulation.

2.   The notifiying entity shall submit the notification in a manner which enables storage of information in a way accessible for future reference and which allows the unchanged reproduction of the information stored.
